The Illinois Premise Alert Program provides for Public Safety Agencies in the State of Illinois to save pertinent information which can be used by police, fire and EMS personnel in the event of an emergency. If an emergency event occurs, firefighters or paramedics are able to access the necessary medical information from their records, prior to arrival at the residence.

The Illinois Premise Alert Program (Public Act 96-0788) was enacted to ensure that consistently high levels of public safety services are available to all people including those with special needs and disabilities. It is the intent of this program to offer guidance and direction to public safety workers in responding to and assisting those individuals with special needs or disabilities or both with whom they will have contact in providing service to.

The ability to effectively deal with special needs individuals is enhanced with knowledge or information about the individual. The information provided will be kept confidential and used only to provide emergency services personnel with the information needed to effectively handle an emergency involving a special needs or disabled individual. The information will expire two (2) years after the date submitted and will no longer be maintained in the database.

The Marengo Fire/Rescue District shall not be subject to civil liabilities for duties relating to the reporting of special needs or disabled individuals and the information provided to the Premise Alert Program will not result in any type of preferential treatment to the special needs or disabled individual.

For the purposes of this Program “disability” and “special needs individual” are defined as:

  • “Disability” means an individual’s physical or mental impairment that substantially limits one or more of the major life activities; a record of such impairment; or when the individual is regarded has having such an impairment.

  • “Special needs individuals” means those individuals who have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition and who also requires health and related services of a type or amount beyond that required by individuals generally.

You may enroll, update or renew the information at any time by completing and submitting a new form. The premise alert program registration form can also be picked up at one of the District’s fire station OR phone requests for the form can be made by calling (815) 568-8912, Monday – Friday, between the hours of 8:00AM – 4:00PM.
